1.)  The type of paper I use is Bienfang Bristol paper.  It comes in 20 two ply sheets.  This type of paper is much more durable than sketch paper as you may erase as much as you wish without damaging the paper.  The sizes I use are as follows:  9" x 12", 11" x 14", 14" x 17", and 19" x 24.

2.) The types of pencils I use are as follows:   Staedtler Lumograph HB to 8B, and Charcoal 3B.  I occasionally use a black fabric marker to mark the pupils of the eyes and nostrils.

3.) Yes, these are the most common pictures that I have commissioned.   It does not matter if one is, for example,  5" x 7" and the other is 8" x  10",  I can match them up and make it look as if they are sitting in the same picture.  One thing that really helps the picture look more realistic (when done this way) is to make sure the light source is coming from the same direction in each picture.  (This way it does not look as if the light is coming from several different directions, resulting in a very unrealistic appearance).

5.)  A 'subject' can be anything.  A person, an animal, a car, a boat, etc.   Whatever you want to put in your picture is a subject.  (I will draw almost anything as long as photographs are provided).
